ANTI-AGING STORE RETURN INSTRUCTIONS
- To ensure proper processing, all returns must include a Return Authorization (RA) number. Contact our Customer Service Representatives at 866-896-0471 to request your RA number. Requests for refunds after 30 days cannot be honored.
- Please complete the bottom portion of this form and include it in the package being returned.
- PLEASE NOTE: Your RA number MUST be clearly written on the outside of your shipping box. Boxes missing an RA number will be refused and returned to sender at sender’s expense.
- Ship via UPS or Certified Mail to: Anti-Aging Store 11701 E 53rd Ave Denver, CO 80239.
- The package must be postmarked within 7 days of RA number issuance.
- For your protection, you may wish to get a confirmation of receipt from the USPS or UPS. Please retain your receipt. No COD’s.
- You refund will be issued in the manner in which you originally paid. If you paid by credit card, a credit will appear within 1-2 statements. Please allow 4-6 weeks from the date you ship your return package for check refunds.
